| Appointments |

All appointments and calls are placed through our main office in Bloomfield at (860) 243-1414, or toll free at (877) 243-1414.
Appointments are scheduled in advance with the physician at the office of your choice. New Office hours are 8:00am to 5:00pm monday through Friday.
In the event of a cancellation, please try to notify the office 24 hours in advance so that we may give the time to another patient.
We try to see all of our patients on time and request that you extend us the same courtesy. Due to the unscheduled nature of emergencies, occasional delays in the appointment schedule do occur. We hope you understand that these delays are unavoidable and that in an emergency you will be extended the same consideration.
At the time of your appointment please remember to bring the following:
X-rays of injury if taken elsewhere- otherwise we have the ability to take them in our office if the physician requires them.

| Phone Calls |

If you have a medical question during regular office hours, our office staff will assist you in either receiving an answer or have you speak directly to your physician. Physicians return patient calls as time permits, either between patient visits or at the end of the day. All emergency calls are answered immediately.
If an injury occurs after regular office hours, our answering service will contact the physician on call. To reach us after hours call (860) 243-1414.

| Prescriptions |
|
Prescription refills should be requested weekdays during the hours of 9:00am and 4:00pm. Refill requests cannot always be accommodated outside of regular office hours especially if your physician is not the physician on call. When requesting a new medication please inform the staff of any allergies to medications and have the name and phone number of your pharmacy ready.

| Insurance |
|
We do participate in most insurance programs. We do ask, however, that you make sure your referral, if required, is still valid each time you are seen. Unfortunately, when a referral cannot be obtained we will be unable to treat you and will reschedule your appointment once a referral is obtained.
Bring your insurance card with you at each visit to assure the correct insurance company is billed.

| Billing |

All correspondence is to be sent to the main office in Bloomfield.
We are happy to assist you in filing for your insurance benefits. However, you are personally responsible for your bill and will receive a monthly statement of your account even if your insurance claim is pending.
We do not wait for legal suits to settle before receiving full payment of your account. We will respond to all attorney requests or insurance company requests, with your permission.
As part of your agreement with your managed care company, your copay is expected at the time of your visit. We accept cash, personal checks, VISA, Mastercard and American Express.
